Please collect and prepare and send the following documents:

1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.

2. Three Passport Photos. They must be 2x2, color or black and white with a plain light or white background and show a full front view of face.

3. Three copies of the Visa Application Form (signed and completed)

4. Please Fill Out the American Passport Express Customer Information Form

5. Proof of vaccination for yellow fever.

6. Copy of Round trip Tickets, Itinerary or confirmation from a travel agent.

7. Processing Fees:

One Money Order made payable to the Consulate General of Burundi (see table below) Do not send personal or company checks. Include your name and date of birth on the money order. Do not include your address on the money order.

One check or money order made payable to American Passport Express for:

Standard Visa Processing $49- leaving in more than 15 days
