Please collect and prepare and send the following documents:
1. You must send your valid US Passport with at least 6 months of validity left and one blank visa page.
2. Two Passport Photos. They must be 2x2, color or black and white with a plain light or white background and show a full front view of face. (Signed on the reverse)
3. Two copies of the Visa Application Form (signed and completed)
4. Important: You must fill out the American Passport Express Customer Information Form
5. International Certificate of Vaccination for Yellow Fever.
6. Copy of round-trip tickets, itinerary or letter of confirmation from your travel agent.
7. Copy of recent bank statement or letter from bank verifying sufficient funds.
8. If visiting friends or relatives in Cameroon (Social Visit), a letter of invitation bearing the seal of the Cameroon Ministry of Immigration is also required.
9. Processing Fees:
One Money Order made payable to the Consulate General of Cameroon (see table below) Do not send personal or company checks. Include your name and date of birth on the money order. Do not include your address on the money order.
