The Department of State requires that you fill out the government application online for expedited service.
Download and fill out the DS-82 Passport Renewal form now- (Do not use pen with red ink.)
State Department regulations state that the application (DS-82) has to be submitted within 5 days of when it is dated. Please date your application for the day American Passport Express will receive your materials. Please call 1-800-455-5166 with any questions. |

B.
Reserve Processing Space & Letter of Authorization
|
Please contact American Passport Express at 1-866-663-7293 to reserve your express processing space.
You will be directed to print two copies of the appropriate letter of authorization (L1, L3, L4, L6, L10) listed below. The letter of authorization gives us permission to process your passport application.
The two copies of the letter of authorization must:
- Have original signatures by the passport applicant
- Both be signed using a blue ink pen if possible
- Not be photocopies or faxes

Most Recent Passport |
Issued within the last 15 years and undamaged. This passport will be returned with your new one. |

Photos must meet the government specifications see photo requirements here or on the backside of your DS-82 form. We strongly recommend not sending digital photos. Where can I get my photos?
(Please do not staple the photos to the DS-82 form.) |

E.
Documentation If Your Name Has Changed |
If the name you are currently using differs from the name on your previous passport you must submit a marriage certificate, divorce decree or court order showing the change of name. The name change document must bear the official seal of the issuing authority. Uncertified copies or notarized documents can not be accepted. All documents will be returned to you with your passport.

Proof of Departure |
Send one of the following:
- Two copies of your plane ticket showing departure date or
- Two copies of your itinerary from the travel agent or airline

Service Fees |
Personal checks, money orders or use the customer information form (see step 2 below) to pay all fees by credit card.
1. Government expedite fee made payable to "US Dept. of State" for $135 (write your Date of Birth on this check.) Click Here for more information about this fee.
2. American Passport Express Service Fee.
Made payable to "American Passport Express" for $70.
3. Return Shipping Fee Return shipping of your passport (choose one):
- Include a prepaid FedEx airbill or
- Add $20 to the American Passport Express fee for overnight shipping

All applicants must complete and submit the customer information form.  After submitting the form online print a copy of the confirmation page which appears. The form gives our company information which is used to help accurately process your passport order. |

When you have all of the items in steps 1 & 2 listed above, call American Passport Express (1-866-663-7293) for the Address to Send your package.
Please send the following via FedEx:
- Completed DS-82 Form [ step 1a ]
- Letter of Authorization signed in blue ink. [ step 1b ]
- Passport [ step 1c ]
- Two Identical Passport Photos [ step 1d ]
- Name Change Document (if necessary) [ step 1e ]
- Proof of Departure [ step 1f ]
- Service Fees [ step 1g ]
- Customer "Confirmation Page" [ step 2 ]

You must call for the shipping address by 10:00 Pm EST M-Th and 8:00 Pm EST Friday.
On Your FedEx airbill Mark the box that says "Priority Overnight" (Next Business Morning)
 Send Using FedEx ONLY
You will receive email confirmation with your order number and status the day we receive your application.

*You can pick up FEDERAL EXPRESS airbills and envelopes at FedEx boxes and FedEx offices. Call 1-800-GO-FedEx to find the most convenient location or use their online drop-off locator (drop-off locations will have blank airbills and envelopes. You may use a credit card to pay for the sending of your application.)
