New Application for US Passport

You are considered a new or first time applicant if you answer yes to any of the following:

You are applying for a US passport for the first time.

Your expired US passport is not in your possession.

Your previous US passport has expired and was issued more than 15 years ago.

Your previous US passport was issued when you were under 16 and you are now over 16.

All applicants under the age of 16 see the "Child under 16 instructions" If your passport is lost, stolen or damaged, and you are 16 or older, see the Lost or Stolen Instructions.

Renewal of Passport

If you answer yes to all of these questions then you are renewing a passport.

I can submit my most recent passport.
I was at least 16 years old when my most recent passport was issued.
I was issued my most recent passport less than 15 years ago.
I use the same name as on my most recent passport, OR, I have had my name changed by marriage or court order and can submit proper documentation to reflect my name changes.
My current passport is undamaged.

Applicant Under 16 Years

All applicants under the age of 16 you follow these instructions.

Lost or Stolen Passport

If your passport was stolen, lost or damaged and would still be valid then you need Instructions for a Lost or Stolen Passport. If you are unsure whether the lost passport would still be valid please use these instructions.

Changing the Name on a Valid Passport

If you can answer yes to the following questions then use the name change instructions:

1. Your most recent passport was issued less than 1 year ago.

2. Your most recent passport is undamaged and in your possession

3. You have legal name change documents to support the change

If you answered NO to any of the questions then click here.

You will submit evidence of the name change such as a marriage certificate, a certified court order or other satisfactory evidence. Traveling with conflicting names on your ID, passport and plane tickets can cause problems so updating the US passport is important.

Adding Visa Pages

If you have run out of visa pages then you need to follow these instructions. Your valid passport can have 24 to 96 new pages added. See Adding Passport Visa Pages

Extending A Limited Passport

If your current passport was only issued for 1 year or less you may be allowed to extend its validity to the full 5 or 10 years. Extension of limited passports usually requires the submission of sufficient proof of citizenship and ID. See Extending a limited passport.
