Proof of Name Change
 

If the name you are currently using differs from the name on your previous passport or birth certificate, you need to show your drivers license to the acceptance agent. If your drivers license does not show your current name or was issued less than six months ago then you should submit a name change document, for example, a marriage certificate or court order showing the change of name. The name change document MUST bear the official seal of the issuing authority. Uncertified copies or notarized documents will not be accepted. All documents will be returned to you with your passport. If Using a Divorce Decree. If your divorce decree does not show your maiden/former name which you intend to use, your birth certificate should be submitted with the application. Identifying documents bearing your new name should also be presented.
