1. Your passport was issued more than one year ago

2. Your passport is not damaged -If its damaged then apply as a first time applicant

3. You have the legal name change documents -If no documents then apply as a first time applicant

4. You were over 16 when your passport was issued -If you were under 16 and are now over 16 then then apply as a first time applicant

5. Your passport was issued less than 15 Years ago- If more than 15 years, then apply as a first time applicant
