Police Clearance Form
To apply for a police clearance from the Houston Police Department submit the following documents:
- HOUSTON POLICE CLEARANCE APPLICATION FORM/ HOUSTON POLICE CLEARANCE ORDER FORM
- COPY OF VALID ID
A photocopy of your valid driver's license, Texas identification card, birth certificate or passport. Expired licenses, identification cards and passports will NOT be accepted.
- FINGERPRINTS (If applicable)
If you have ever been arrested by the Houston Police Department, you must include a legible set of fingerprints.

$20.00 per application
- SERVICE FEE
$40.00 per application - 3 to 4 business day processing
$60.00 per application - Same day processing
