To qualify for a passport renewal, you must be able to answer YES to all of the following conditions:
- I am a U.S. citizen
- I can provide my most recent passport
- I was at least 16 years old when my most recent passport was issued
- I was issued my most recent passport LESS than 15 years ago
- I use the same name as the name reflected on my most recent passport OR I have changed my name and can submit a marriage certificate or court decree to verify this change
To qualify for a first time passport, the applicant must:
- Provide proof of U.S. citizenship (such as a birth or naturalization certificate)
- Have a government-issued photo ID (such as a driver's license or military ID)
- Be over the age of 16
- Be able to provide proof of name change if your name is different than the one on your old passport (marriage certificate or court order)
To replace your lost or stolen passport, you must be able to answer YES to one of the following conditions:
- My valid U.S. passport has been lost or stolen
- My expired U.S. passport has been lost or stolen
To change the name on your passport, you must:
- Submit your original marriage certificate or court decree as evidence of your name change
To replace your damaged passport, you must be able to answer YES to one or more of the following conditions:
- My valid U.S. passport is frayed, torn, separating, or altered in some other way
- My valid U.S. passport has been deemed unsuitable for use by U.S. immigration authorities
To qualify for a child's passport, the applicant must:
- Be under 16 years of age
- Have consenting parents or legal guardians willing to complete a passport application on their behalf
- Be able to appear in person with their parents or legal guardians to complete their passport application process
To obtain a Second Passport, evidence of one or more of the following must be present:
- Consecutive dates of international travel causing conflict with normal visa processing procedures
- Planned entrance into certain Arab Nations or Israel
- Multiple visa stamps prohibiting travel within certain Arab Nations or Israel
- Documented need to travel between certain Arab Nations or Israel
