Here is a list of the different forms. Select the one(s) that will apply to your application. Complete each one and provide the documents instructed by the form. Most supporting documents may be the original, certified copy, or notarized.

Form DS-11 Application For A U.S. Passport

Complete Form DS-11 if:

  • you will apply for the first time
  • you will apply for a minor or child
  • it expired more than five years
  • it was lost or stolen
  • you need a second passport
  • you need a card

Form DS-82 U.S. Passport Renewal Application For Eligible Individuals

Complete Form DS-82 if:

  • your passport will expire soon
  • it expired less than five years
  • you still have your passport, and it is not damaged, lost or stolen
  • you were sixteen or older when you received it
  • no name change was done
  • you will get a second passport

Form DS-64 Statement Regarding a Lost or Stolen U.S. Passport Book or Card

Complete Form DS-64 if:

  • your document was lost or stolen

Form DS-5504 Name Change, Data Correction, and Limited Passport Book Replacement

Complete Form DS-5504 if:

  • you changed your name less than a year
  • your information was misprinted
  • you have one that is limited to two years or less

Form DS-3053 Statement of Consent

Complete Form DS-3053 if you are applying for a minor and:

  • only one parent or guardian signs Form DS-11
  • absent parent or guardian must complete this form

Form DS-5525 Statement of Exigent/Special Family Circumstances

Complete Form DS- 5525 if you are applying for a minor and can provide proof than:

Government Agency Fee
