Fill out Form DS-11

  • You should provide your SSN (Social Security Number). If you do not have one, a signed and dated statement should be included in your application including the phrase “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct:  I have never been issued a Social Security Number by the Social Security Administration.”
  • If you want to ask for a larger passport book (52 pages), you will find a box at the top of the Form DS-11. Check it.

Proof of US Citizenship

You have to submit an original or certified copy of one of the following documents. Certified copies mean that the papers include the seal or stamp of the official issuing authority:

  • Valid U.S Passport (It can be expired, but it must be undamaged).
  • U.S Birth Certificate, including the following requirements.
  • It must be issued by the city, county, or state of birth.
  • It should include your full name, date, and place of birth, and your parent(s)’ full names.
  • It must be signed by the registrar and includes the date filed with the registrar’s office and the seal of the issuing authority.
  • Consular Report of Birth Abroad or Certification of Birth.
  • Certificate of Naturalization.
  • Certificate of Citizenship.

Photocopy of the U.S Citizenship Proof:

You should submit a photocopy of the front and back of the U.S citizenship. The photocopy must follow the following points:

  • It must be legible.
  • It must be on white 8.5”x11” standard paper.
  • All the photocopies must be single-sided and black and white.

Some people do not submit photocopies of their U.S citizenship. They send second certified copies of their U.S citizenship. It is necessary to attach either a photocopy or a second certified copy. Otherwise, the Department of State will not process your application and your passport will be delayed.

Present a clear photocopy of your ID. The copy must be clear with the front and back of the photo.
